Your child isn't lazy, and you didn't miss something obvious
Here is the pattern almost every homeschool parent hits. Math was fine in second grade. Fine in third. Somewhere in fourth it stopped being fine, and now every lesson takes forty minutes and ends in tears, and you cannot point to the day it changed.

The three-step method, stripped down
Step one: start lower than feels reasonable. If you begin at your child's grade level, you find out whether they can do grade-level work — which you already suspected. You learn nothing about the foundation. Start several years down and let them fly through the easy part.
Step two: walk the prerequisites, not the checklist. Skills have dependencies. Long division needs subtraction facts and place value; fractions need division. When a child misses, the useful next question is "which skill underneath this one is weak," not "what's the next question on the list." Step three: require mastery to move on. A gap that's found and then not closed is just a documented gap. Khan Academy has no placement assessment — you pick where to start, which means the gap-finding is still yours to do. IXL is drill-only: plenty of practice, no teaching and no placement.
